I, hereby give permission for my son/daughter to be photographed or videotaped by Christ the King Catholic Church ("Parish") and the Archdiocese of Oklahoma City (the “Archdiocese”) and consent to the use by the Church and the Archdiocese of any videotapes, photographs, slides, audiotapes or any other visual or audio reproduction produced by the Church and/or the Archdiocese, including my child’s name (the “Materials”).
