OLOG Liability Release

I,

for and in consideration for the use of the Camp OLOG property (“the Property”) owned by the Archdiocese of Oklahoma City ( “Owner”) do hereby:

1. Waiver and Release: release, forever discharge, and hold harmless the Owner, and its successors, assigns, affiliates, directors, officers, employees, and agents, from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the my use of the Property. I acknowledge that I am he releasing the Owner from any and all liability for injury, illness, death, or property damage resulting from my use of the Property the fullest extent permitted by law.

2. Assumption of Risk: I hereby acknowledge the risk of using the Property and hereby expressly assume any and all risk of injury or harm associated with such use.

3. Indemnification: I agree to hold harmless and defend the Owner against any and all actions, claims, expenses, or demands arising therefrom that may be made or brought for any injury to third parties arising out of my actions or omissions arising from the use of the Property, including but not limited to reasonable attorneys’ fees and expenses.

4. Covid-19 and other Infectious Diseases: I hereby acknowledge that the risks associated with transmission of COVID-19 remain high, and the Owner cannot guarantee a risk-free environment at the Property. Additionally, I acknowledge that the Owner cannot guarantee that guests follow social distancing, hand sanitization, and personal hygiene guidelines. Therefore, by signing this form I specifically acknowledge that the Waiver and Release (Section 1) and Assumption of the Risk (Section 2) of this Release specifically include illness or death resulting from Covid-19 or other infectious diseases.

5. Minors: I hereby agree and understand that all provisions of this Release, including but not limited to, all releases of liability and assumptions of the risk, are also applicable to any minors under my care. I assume full responsibility and supervision of any such minors.
